Autumn Care of Suffolk is a 120 bed skilled nursing and rehabilitation facility. We have been serving the Suffolk area and surrounding communities with quality long-term care for over 25 years. We offer short-term rehabilitation, long-term care, and respite stays.
Our rehabilitation services include physical, occupational, restorative and speech-language therapies. Our therapy staff consists of 7 full time therapists with a combined rehabilitation work experience of over 70 years.
Our dedicated staff of qualified nurses and nursing assistants ensure our residents get the highest quality of care as well as plenty of TLC. Skilled nursing services include clinically complex care management such as wound care, tracheotomy care and IV antibiotic therapy. We also offer hospice services to our residents suffering with terminal illness.
Autumn Care of Suffolk works in collaboration with the many local community groups and religious organizations, whose members come to the facility on a regularly scheduled basis to provide church services and activity programs. We have two facility vans which transport residents to scheduled medical appointments as well as providing transportation to community events and recreational activities.
As methods of payment, the facility accepts, Medicare, Medicaid, Managed care contracts, VA contracts, long-term care insurance, and private funds.
